Franky Rook

Franky Rook has served as a Sr. Care Manager with Life Changes since March 2016. She has extensive experience working with the elderly, those with PTSD, mental disabilities, substance abuse, and dementia. She is knowledgeable and intuitive when it comes to assessing the needs of her clients and developing care plans. She is a natural at creating strong relationships. Her extensive experience in the medical field provides expertise in navigating the health care systems as well as facility nursing, and related services. Franky uses her artistic talents and knowledge of the aging process to create effective and thoughtful care plans. She has a bachelor equivalent degree in nursing from SROV Medical School in Holland and studied art at VLVU University of Amsterdam. She has U.S. credentials as a Certified Nursing Assistant, Medication Technician, Activities Director, and Phlebotomist. Franky is a wife, the mother of three, a grandmother, and an artist of exceptional talent.
